Know an Awesome Dad? Nominate Him for Father of the Year

PAPÁs Fatherhood Awards will be presented later this month.

Do you know the normal duties of fatherhood for his children, family, and community?

Then nominate him for the 2012 PAPÁS Fatherhood Awards! Three outstanding Santa Cruz County fathers or father figures will be chosen to win the Father of the Day, Father of the Month and Father of the Year Awards at the 6th Annual on Sunday, June 17.

Nomination Guidelines and Requirements
The PAPÁS Fatherhood Awards recognize local fathers or father figures who go to extraordinary lengths to keep their children, family, and community connected and balanced.

To nominate a local father or father figure, complete this form and submit it with a typed or legibly handwritten nomination of no more than 1,000 words that describes the father/nominee, why he should be considered for a 2012
PAPÁS Fatherhood Award, and answer the following questions about him (if applicable):

  1. Ongoing commitment and dedication to his child(ren).
  2. Extraordinary efforts as a father and co-parent with the mother of his child(ren).
  3. Efforts to successfully balance work life and family life.
  4. Attempts to mentor and strengthen other fathers, father figures, and/or children.
  5. Unique qualities or stories about the nominee that makes him stand out as an exceptional father.

Self nominations will not be accepted. All nominees must be active fathers or father figures who live in Santa Cruz County. 2012 PAPÁS Fatherhood Award recipients will be chosen by Community Members identified by the PAPÁS Program.
